Touro at Home Care Plan

Touro At Home services are provided by healthcare professionals under the direction of the patient's personal physician. When a physician recommends home-care services to aid a patient's recovery, a nurse visits the home to assess the patient's medical needs in the home environment and to determine the family's ability to participate in care. A care plan is then established based on the doctor's orders. The amount and type of services provided varies according to the patient's needs and the insurance plan involved and may include the following: 

  • Skilled nursing
    Professional registered nurses perform a variety of specialized and highly skilled duties. Touro At Home nurses are the case managers — they teach and advise patient and family members, coordinate the treatment plan with other team members and communicate with the patient's physician.
  • Physical therapy
    Licenesed physical therapists implement rehabilitation programs designed specifically for each individual patient. Programs may include therapeutic exercise and instructions on the use of crutches, canes or walkers to improve ambulation or to maximize physical functioning.
     
  • Occupational therapy
    Licensed occupational therapists provide treatment for improving or restoring functions impaired or lost because of illness or injury. The occupational therapy program includes training to improve self-care and activities of daily living.
     
  • Speech pathology
    Certified speech pathologists assess and treat patients with speech and language disorders which result in communication difficulties.
     
  • Medical social workers Counseling
    Medical social workers provide short-term counseling and support to the patient in dealing with the lifestyle changes due to illness or injury. Social workers also assist patients with referral and utilization of appropriate community resources.
     
  • Home health aide
    Home health aides work under the direct supervision of a registered nurse or therapist to provide services related to the patient's personal care. Aides are available to help with bathing, meals, exercise and ambulation. Certain light household services, essential to healthcare at home, may also be provided.
     
  • Other available services
    Touro At Home is available to provide or assist in providing other home care services. Specialty nursing services include chemotherapy, home IV and nutritional therapies. The provision of medical supplies, equipment and laboratory services can also be coordinated by Touro At Home.
